I've had several instances of ghosting at the end of a wave and the game kills me just prior to the new wave. Is this normal?

yup. It's a fix for something worse that happens if your ghost continues...

Basically, the pawns diverged. Once you recovered from ghost, you had one pawn over which you had no control that took damage. Then you could run around...until a monster found your invisible self. Then you were dead
